Informed Consent & Assumption of Risk

I, the parent/legal guardian of the child(ren) named on this form, give permission for my child(ren) to participate in Little Giants Basketball Clinic sessions organized by Momentum Sports Africa (including at Don Bosco Upperhill and associated venues).

I understand that participation in sports activities involves inherent risks, including but not limited to physical injury, illness, and other unforeseen circumstances. I voluntarily assume all such risks associated with participation.

Release of Liability

To the fullest extent permitted under the laws of Kenya, I release and hold harmless Momentum Sports Africa, their directors, officers, staff, contractors, coaches and volunteers, partner schools, facilities, sponsors and affiliated organizations from any claims arising out of participation in the clinic, except in cases of gross negligence or willful misconduct as defined under Kenyan law.

Medical Authorization

In the event of injury or emergency, I authorize Momentum Sports Africa staff and designated representatives to obtain medical treatment for my child as deemed necessary by a licensed medical professional. Reasonable efforts will be made to contact me unless immediate treatment is required.

Data Protection & Information Sharing

Personal data will be processed in accordance with the Data Protection Act.

I consent to participant information, photographs, video recordings and related data being used by Momentum Sports Africa for program administration, safeguarding, communication about clinic schedules and similar programs, and evaluation where applicable.

Data will not be sold or shared for unrelated commercial purposes.

Media Consent

I grant Momentum Sports Africa permission to capture and use photographs and video recordings for documentation and promotional purposes related to the clinic and Momentum Sports Africa activities. The participant's full name will not be publicly disclosed without additional consent.

Code of Conduct

Momentum Sports Africa reserves the right to remove any participant for misconduct, disciplinary violations, or behavior that compromises safety or program integrity.
